Instructions for Requesting a Rotation: 3 Steps
Step 1: Complete a Rotation Request Form (do not hit enter/return until ready to submit)

STEP 2: Preliminary DocumentsAfter submitting the Rotation Request Form, please submit the following:
*Once your Rotation Request, Photo and CV are received, you will be notified via email if your rotation can be accommodated.
STEP 3: Required Documentation
Once your initial rotation request is approved, you will be asked to submit the following:
A) Application Packet:
-Rotation Request from your medical school
-Official COMLEX I & II Score Report
*If COMLEX Step II not yet taken or scores not yet released, please note when you have or will take the exam and submit scores when available
-Letter of good standing from Dean's Office
-Proof of Liability Insurance
-Immunizations, ppd, and flu shot (please make sure your ppd and flu shot is within 12months of your expected rotation dates)
*Please have all documents of the application packet scanned and sent in a single email, preferably as one PDF file, to [email protected]
*Please name the file with the rotating students name (Example: JoeSmith.pdf)
B) Affiliation Agreement:
-Agreement must be approved 60 days prior to the start of your rotation; We recommend submitting 90 days prior to rotation
-Your Medical School's Rotation Coordinator must contact the GME department at JCMC to request an affiliation agreement
*Note: If we do not receive the completed Affiliation Agreement and Application Packet within 30 days of notification of rotation scheduling, your rotation will be forfeited
Medical Student Orientation:
After your rotation is confirmed, you will be sent an email with instructions regarding orientation, including information about housing, parking, hospital policies, where to go on your first day, computer access, ID badges as well as the student google drive access for the medical student handbook.
The JCMC Orthopaedic Surgery Residency Program participates in Osteopathic ERAS
We require the completed ERAS application, a Dean's Letter (MSPE), at least 3 letters of recommendation, official medical school transcripts, COMLEX Steps I & II and highly recommend rotating with the program.

Students who rotate with us will be given preference for interview selection.
Select students who have not rotated with us but have scored >550 on COMLEX Steps I & II and have shown superior performance in their medical school courses, research and in their letters of recommendation, may be offered interviews.
